Why were you banned?: I got CT banned for a month for turning on friendly-fire and killing a teammate as a CT. A little bit over a month later I logged onto the server on a different account without realizing I had not swapped back to my main CS account (Keep in mind I was playing T the entire time), and I got perm CT banned by console for evading after I tried to end a round by swapping myself to kill.
Also it says I evaded with two accounts, one of the accounts linked to my ban isn't even mine.
Why should you be unbanned?: I have since learnt from my actions and will make sure NOT to break any rules and not use any other accounts when I play the server.

I have unbanned your main and your main only.
If you get CT banned dont try and evade again
