From my guess it's IP ban first then if the person comes back it's HWID Ban or Steam ID

Well basically your computer has a unique ID, called a UNIQ-HWID-CPUID, when you join the server we can see that it is you.
To hide this, you need to move to Alaska where the internet is run through plastic cables rather than the liquid helium we have over here, then we can't track you.
